Prioritizing Social Needs
The Community Connected Care program helps members access evidence-based care, in part by addressing their social needs and removing treatment barriers.
Program staff connects the people we serve to resources through the SydneySM Health platform, by phone or in person. It also provides support through collaborations with community-based organizations, such as food banks, area agencies on aging, and other nonprofits.

“At Equality Health, we embrace a holistic approach to health and healthcare, addressing the physical, emotional, and social needs of individuals and diverse communities. Our partnership and collaboration with Elevance Health has advanced this vision from serving a handful of local members to over 100K Medicaid Elevance members across 4 states. Together, we enhance person-centered outcomes by integrating comprehensive care strategies with a social-first approach and a dedication to cultural humility, all while fostering a collaborative environment that prioritizes individual well-being.”
