Prescription medications play a vital role in maintaining and improving health, especially for people with chronic conditions. That’s why collaboration between prescribing physicians, pharmacists, and health plans is so impactful. Our pharmacy benefit manager, CarelonRx, collaborates with care providers to make pharmacy simpler and more integrated for the people we serve.
Expanding Access to Behavioral Health Care
Mental health is a consideration for everyone, so it must be addressed to drive improvements in whole health. Carelon Behavioral Health uses strategic partnerships, technology, and data-driven solutions to make it easier for care providers to manage behavioral health across care settings. The Carelon Behavioral Health crisis safety platform and Resilience Through Information, Support and Education (RISE) program are two recent examples.

As one of the nation's largest community-based, non-hospice, care provider-led palliative care organizations, Carelon Palliative Care does more than provide end-of-life services.* We add value and efficiency by connecting all the pieces and players that contribute to a person’s overall health, while also providing supplemental care. Not only does this make for higher patient satisfaction and better outcomes, but it also leads to lower overall healthcare costs.
Improving Cancer Care With Data-Driven Collaboration
Cancer exists in many different forms, each with its own unique cause, impact, and treatment. Carelon Health’s specialized models, programs, and solutions help oncology practices and cancer centers coordinate across disciplines to deliver personalized, patient-centered care. Most recently, we enhanced our suite of services with the addition of a new, digital care provider dashboard. It brings together quality and performance metrics to support care providers in driving high-quality patient outcomes and sharing in the success of our Oncology Medical Home value-based care model.
